Usefulness of Low-Viscosity Contrast Media in CT Pulmonary Angiography in Elderly Patients with Weak Blood Vessels

Abstract
Purpose : To identify clinical usefulness and expected effects of using low-viscosity contrast media when performing CT angiography of pulmonary artery on reducing patients¡¯ risks and maintaining the image quality.

Materials and Methods : 39 patients aged 65 and older who had undergone CT angiography of pulmonary artery from May 2011 to December 2013 in Seonam Hospital were selected for the test. In their test injection of physiological saline, psi appeared on the monitor for dual flow injector was 50 and higher. High-viscosity contrast media Xenetix 350 was used in 26 people, and los-viscosity contrast media Pamiray 300 was used in the remaining 13 people in order to evaluate the quantitative and qualitative difference between them. SPSS version 19 was used to verify the statistical significance of the test.

Results : When high-viscosity contrast media was used, psi was between 150 and 250, on the other hand, when low-viscosity contrast media was used, psi was between 100 and 200 which is about 100psi decrease. When high-viscosity contrast media was used, it was combined with the blood in the right atrium and right ventricle, and HU value decreased about 58%, and when low-viscosity contrast media was used, HU value decreased about 57%. After injecting contrast media, it was shown that HU value up to the SVC and right atrium is higher when using high-viscosity contrast media compared to using low-viscosity contrast media. However, it was p=0.684 in the pulmonary artery and p=0.639 and p=0.652 in the right pulmonary artery and left pulmonary artery, respectively. When verification for significance level of 5% was performed for those 3 sections, it was found that there is no significant difference between them. As for the qualitative evaluation, it was found that the average score of the high-viscosity contrast media group of 26 people is about 3.8, and the average score of the low-viscosity contrast media group of 13 people is about 3.6. The qualitative evaluation score was higher when using high-viscosity contrast media but the difference was not great.

Conclusion : It was found that using low-viscosity contrast media when performing CT angiography of pulmonary artery in elderly patients with weak blood vessels can greatly reduce patients¡¯ risks such as extravasation of contrast media while not compromising the areas where diagnosis and examination can be performed. Other than pulmonary artery xamination, if additional studies are conducted for patients with weak blood vessels who require rapid injection of contrast media, it would provide great help to clinical diagnosis.
